By the end of this course:
You will have a good understanding of the Information Sharing processes to support fully integrated working at the early intervention and prevention level of service delivery.
You will know how to make an assessment of additional needs using the CAF form to initiate integrated working to a Team Around the Family (TAF) model and the key pathways to partner agencies.
You will also understand the TAF model, its benefits to the family and how the model can enhance your own role in delivering services.
You will also learn how to carry out the role of Family or Young Person Contact as part of the integrated team and by measuring outcomes effectively, you will contribute to developing improved and seamless service delivery across Powys.
