Your Plan of Care says exactly what help you will get, who will help you, and when. After we have got to know you and what you need (by doing your assessment) and when we have agreed the way ahead, we will confirm what services we can offer you.
If approved, we will provide you with a formal ‘Care and Support Plan’ telling you what will be provided, when and who by. Your service may be provided directly by us or we may pay a private or voluntary sector provider to do it.
Alternatively, you can ask to be paid an equivalent amount of money in cash, so you can buy your own services. This is called a Direct Payment and is readily available but does have some requirements that you will need to meet.
You will then know how much, if anything, your services will cost you and, of course, may talk to us about any changes you want to make.
If you agree with the Care and Support Plan, we’ll start the services as soon as we can, but we may not be able to set everything up immediately.
